As a Service Coordinator, you will be responsible for coordinating personalised supports for participants within the NDIS framework. You will collaborate with families, external service providers, and community organisations to develop, implement, and review individual support plans. You will also lead and mentor a dedicated team of support workers to ensure high-quality service delivery.

  • Participant Coordination: Work closely with participants to assess needs, develop individual NDIS plans, and monitor progress.
  • Team Leadership: Provide supervision, training, and mentorship to support workers, fostering a positive and inclusive team culture.
  • Service Liaison: Build and maintain relationships with a range of service providers, ensuring participants receive the right services.
  • Compliance & Documentation: Ensure all documentation is up to date and compliant with NDIS guidelines and confidentiality standards.
  • On-Call Rotation: Participate in rotational on-call duties, providing guidance in urgent situations.
Qualifications: Bachelor’s degree in Social Work, Psychology, Disability Studies, or a related field (or extensive relevant experience). NDIS Experience: Comprehensive knowledge of the NDIS framework and experience developing individualised support plans. Leadership: Proven experience in supervising and leading teams. Communication: Strong advocacy, cultural competence, and excellent communication skills. Compliance & Risk Management: Knowledge of relevant legislation and experience with risk assessments and budget management. First Aid: Senior First Aid Certificate is essential.
