Our client, Mix It Up – Live life your way is a social connection and engagement service that is focused on assisting people to achieve their individual goals.  Mix It Up – Live life your way is based in Surrey Downs however, work is undertaken in the broader community, providing a variety of services to people receiving NDIS funding. 

We are currently looking for a Lifestyle Consultant/Service Coordinator to assist our Lifestyle Options team. The Lifestyle Options team provide individualised support for people who may have a disability, mental health condition, experience memory loss, are ageing or socially isolated, and accompanies people over the age of 18 to participate in social, community and recreational activities of their choosing.

Reporting to the Lifestyle Options Manager the Lifestyle Consultant/Service Coordinator provides direction, support and supervision to ensure a high quality of service to Mix It Up- Live Life Your Way clients.

  • Support the Lifestyle Options Manager
  •  
  • Meetings with potential and current clients, their families & carers to discuss needs
  •  
  • Develop, plan and implement individualised service plans & risk assessments
  •  
  • Build strong relationships with clients, families & stakeholders
  •  
  • Rostering management including adequate shift coverage
  •  
  • Promote the company’s mission & values within the team
  •  
  • Ensure all compliance measures are being met
  •  
  • Oversee the implementation of client supports and review of ongoing needs
  •  
  • Ability to undertake Lifestyle Companion/Community Support tasks (as required)
  •  
  • Flexibility in the range of hours required to satisfy participant and organisational needs
  •  
  • Provide regular feedback to the Lifestyle Options Manager, director and the NDIS as required
  •  
  • Ability to work with and accept cultural, racial and social diversity
  •  
  • Availability to be on call as required 
  •  

Demonstrated background in mental health, disability or the community services/human services sector

Strong, respectful communicator (Excellent verbal & written communication skills)

Mentoring skills including experience in motivating, developing and directing people in their role while supporting productive working relationships

Leadership qualities including the ability to develop and maintain effective working relationships with colleagues, health practitioners, government departments and other contacts across the sector

Excellent interpersonal & listening skills

Self-driven, dynamic and responsive

Knowledge of the NDIS framework

Understanding of the NDIS pricing arrangements and billable hours accountability

Strong administrative skills; time management, coordination of tasks, balance competing priorities, efficient work practices

High level of attention to detail

Excellent computer literacy

Organised, flexible & responsive

Ability to work with minimal supervision but seek clarification when needed

Proven ability to exercise independent judgement and problem solving with the ability to address issues as they arise

A team player with a ‘Can-do’ attitude

Honest & transparent

Solid understanding of the Mental Health Sector and Community Resources (Desirable)

Provision of satisfactory screening/clearances (NDIS workers clearance or Disability services/aged care & WWCC) or willingness to obtain

‘C’ class drivers licence

Full comprehensive vehicle insurance

Current influenza vaccination

 

  • Minimum of Certificate 4 in the Human or Community services field – essential
