BoraCare seeks a compassionate and experienced Senior Disability Support Services to lead our dedicated team in providing exceptional care and support to individuals with disabilities.

 

    • Champion the voices of individuals with disabilities, ensuring their perspectives are heard and respected in all aspects of care.
    • Uphold the rights of individuals with disabilities as outlined in relevant rights charters.
    • Cultivate cultural humility, safety, and proficiency within the care environment.
    • Collaborate with case managers or relevant support workers to maintain connections with family, relatives, and friends.
    • Facilitate access to community activities and services to enhance resilience and community belonging.
    • Foster self-esteem and individual identity through personalised care and the maintenance of life story books and cultural plans.
    • Provide educational opportunities to support academic achievement, independent living skills, and personal development.
    • Monitor and review care and support provided to individuals, addressing nutrition, clothing, education, and transition planning.
    • Ensure staff are trained and informed about relevant medical issues and medication administration.
    • Support the Manager, Disability Services in staff supervision and development.
    • Attend and support monthly staff meetings and regular care well-being/team meetings.
    • Support thorough transition processes for intake and exit of individuals in care.
    • Liaise with stakeholders through weekly reports and timely responses to requests.
    • Provide post-incident debriefing and support to individuals and staff.
    • Conduct safety audits of medication, house maintenance, staff activities, and property in compliance with guidelines.
    • Maintain accurate and secure documentation of participant information, adhering to privacy legislation.
    • Assist in managing the budget and household requirements in collaboration with line management.
    • Ensure effective communication and reporting processes to meet statutory, organisational, and funding body requirements.
    • Conduct risk assessments for household activities and implement behaviour support and risk planning as needed.
    • Ensure compliance with Occupational Health and Safety guidelines and practices.
    • Establish, lead, coach, and inspire an engaged and productive team, providing support, guidance, coaching, leadership, and empowerment.
    • Lead the team in implementing therapeutic, trauma-informed, culturally aware practices and effective process governance.
    • Participate in regular supervision and performance reviews with line management, promoting collaborative working relationships.
    • Support the Manager in participating in relevant service networks and partnering with other agencies and funding bodies to improve and develop quality services.
  • Certificate IV in Disability, Child, Youth and Family Intervention or Community Services or relevant field, or higher.
  • Knowledge of trauma-informed care, cultural competence, and disability support principles.
  • Strong communication, interpersonal, and organisational skills.
  • Ability to work effectively within a multidisciplinary team and liaise with external stakeholders.
  • Understanding of relevant legislation, policies, and procedures in the disability support sector.
  • Current Working with Children Check and relevant clearances as required by regulatory authorities.
