Our team holds a variety of qualifications in mental health services, disability services, aged care, and community services, with extensive experience in working with people from a person-centred perspective. All workers hold current criminal history screening clearances and have a current First Aid certificate. Workers are also trained in Mental Health First Aid and Cultural Competences.

 

We are looking for a passionate and empathetic Support Co-ordinator and Psychosocial Recovery Coach to join our team. In this role, you will support individuals in navigating their recovery journey, connecting them with appropriate services, and assisting them in achieving their personal goals. You will work closely with clients to develop and implement tailored recovery plans, fostering resilience and independence.

– Collaborate with clients to identify their needs, strengths, and goals.

– Develop and implement personalised recovery plans in partnership with clients.

– Co-ordinate support services and resources, ensuring seamless access for clients.

– Provide ongoing coaching and support to clients, helping them build skills and confidence.

– Monitor and review clients’ progress, adjusting plans as needed to ensure positive outcomes.

– Advocate for clients’ needs and preferences within the broader service system.

– Maintain accurate and up-to-date records of client interactions and progress.

– Stay informed about best practices and new developments in psychosocial recovery and support co-ordination.

 

A minimum of certificate 4 in Mental Health or at least 2 year’s experience in providing mental health services?

– Strong understanding of the National Disability Insurance Scheme (NDIS) and its processes.

– Excellent interpersonal and communication skills, with the ability to build rapport with clients and stakeholders.

– High level of organisational skills and attention to detail.

– Empathy, patience, and a genuine commitment to supporting individuals in their recovery journey.

– Ability to work independently and as part of a team.

– Proficiency in using digital tools and platforms for record-keeping and communicationA
